Integrating Coherence Therapy and Transactional Analysis: A Clinical Case Study of an Adolescent with Type 1 Diabetes

Abstract

Adolescence poses specific challenges for managing type 1 diabetes, and difficulties with treatment adherence often carry relational and emotional meaning. This paper presents a clinical case illustrating an integrated psychotherapeutic approach that combines Coherence Therapy (CT) with Transactional Analysis (TA) in the work with an adolescent living with type 1 diabetes and marked non-adherence. CT is used to elicit and make explicit the implicit emotional learning that renders the symptom coherent and self-protective, while TA provides a clinical framework to work with ego states, script beliefs, and redecision processes. The integration supports a stable shift in meaning, leading to improved treatment adherence, greater autonomy, and reduced family conflict. The case highlights the potential clinical value of CT-TA synergy for adolescents living with chronic illness.
